Chapter One: Introduction
1.1 Background of the Study
The National Health Act (NHA), passed in 2014, provides a legal framework for improving healthcare delivery in Nigeria. The Act aims to ensure universal health coverage, improve the quality of healthcare services, and increase funding for the health sector. However, the implementation of the NHA, particularly in rural areas, remains a challenge due to limited infrastructure, poor access to healthcare services, and inadequate funding (Mohammed & Ibrahim, 2023). In Adamawa State, the local government plays a crucial role in implementing national health policies, especially in rural areas where access to healthcare is limited. Local government authorities in Adamawa have been tasked with executing health programs aligned with the NHA, such as providing basic healthcare services, promoting preventive health measures, and ensuring the distribution of healthcare resources (Sadiq & Chukwuma, 2023).
This study examines the role of local government in the implementation of the NHA in rural areas of Adamawa State, evaluating the effectiveness of local government policies in addressing health challenges and improving healthcare access.
1.2 Statement of the Problem
While the National Health Act provides a comprehensive framework for improving healthcare in Nigeria, the implementation of its provisions in rural areas of Adamawa State has been inadequate. Challenges such as insufficient funding, inadequate health infrastructure, and lack of trained healthcare workers continue to impede progress (Yakubu & Bello, 2023). This study will investigate the role of local government in implementing the NHA in rural Adamawa, assess the effectiveness of its strategies, and identify barriers to successful implementation.
1.3 Objectives of the Study
1. To evaluate the role of local government in implementing the National Health Act in rural areas of Adamawa State.
2. To assess the effectiveness of local government policies in improving healthcare access in rural areas.
3. To identify challenges faced by local governments in implementing the NHA and propose solutions for improvement.

1.5 Research Hypothesis
1. Local governments in Adamawa State play a significant role in implementing the National Health Act, but their efforts are limited by insufficient funding and inadequate infrastructure.
2. The implementation of the NHA in rural areas has had a positive impact on healthcare access, but challenges remain in ensuring sustainable improvements.
3. Addressing challenges such as inadequate healthcare infrastructure, funding, and training will improve the implementation of the NHA in rural areas.
1.6 Significance of the Study
This study is significant as it will provide insights into the role of local governments in implementing national health policies and the challenges faced in rural areas. The findings will help local government authorities, policymakers, and healthcare stakeholders to develop more effective strategies for improving healthcare delivery in rural Adamawa.
1.7 Scope and Limitations of the Study
This study will focus on the role of local government in implementing the National Health Act in rural areas of Adamawa State. It will evaluate the effectiveness of local government policies and the challenges encountered in implementing healthcare programs. Limitations include potential biases in responses from local government officials and difficulties in obtaining data on healthcare outcomes.
1.8 Operational Definition of Terms
• National Health Act (NHA): A legal framework established to provide guidelines for improving healthcare delivery, increasing funding, and ensuring universal health coverage in Nigeria.
